Your child may not yet demonstrated academic proficiency, as identified on the most recent assessment data. In order to provide additional instructional support, we have scheduled your child to receive tutoring services with TutorMe.com a District approved vendor, in the following subject area(s): Mathematics.

Your child may access instructional resources related to this tutoring support through their District-issued device by signing on through Schoology.  By completing this form, you represent that you have the legal authority to execute this consent and release form on behalf of your child and you consent to tutoring help for your child provided in person or virtually.  You further acknowledge and agree that you and your child will abide by the District’s Responsible Use Policy.  Additionally, by signing below, you hereby release and hold harmless the Los Angeles Unified School District and its authorized representatives from any and all actions, claims, damages, costs, or expenses, including attorney's fees, which relate to or arise out of the tutoring services described above.  Please complete the information below and click submit.
