You will also be asked to provide a copy of your organization's: (1) bylaws, (2) Articles of Incorporation, and (3) any current COVID protocols, if you have them. These documents will help the volunteers better tailor their advice to your specific situation.

Please note: Should your organization be selected for participation in the clinic, your executive director, board chair, or other senior staff or board member responsible for governing or managing your organization must be able to participate in the clinic.
