Thank you for your interest in our preschool screening process for your child.  Please fill out this form completely to request a screening packet.  Once received by District 126, your address will be checked to ensure it is located within our district boundaries.  If it is not, you will be contacted by the Department Secretary to provide the correct school district for a preschool screening for your child.

Please note: Child MUST have turned 3 years of age in order to receive a packet to be screened.  If your child is not yet 3 years of age, please wait until the month of their birthday to complete this online form.  

Once you complete and return your packet, you will be contacted to schedule a screening.  Screenings are held at least twice per trimester and in the summer, depending upon the number of packets received.  

Screening for the program does not guarantee a classroom placement.  Eligibility must be met based upon state requirements for Preschool For All.
