OVERVIEW: This informal research project seeks to gather various personal perspectives on the experience of love toward the creation of art-based “reports” including songwriting and other intuitive collation methods. 

NOTE TO PARTICIPANT: In filling out this survey data collection form you consent to having your perspectives alchemically transmuted into art. Please ensure to omit any reflections you would not like to have included in a song, a paper magazine, book, poster or any other creative format. Wherever and whenever possible, identifying details will be redacted, with the exception of handwriting. Note that handwriting samples on paper surveys might be included in various formats including website or paper publishing (i.e. scanned or photocopied and reproduced for Art) or other above-listed creative format. Please note that by filling out this survey, your perspectives are not guaranteed to be included in the resulting Art report formats or may be unidentifiable.
