PERMISSION TO JOIN MCJROTC -  I hereby request that my child, be permitted to participate in the Marine Corps JROTC program for school years 2020-2021. I understand the activities may include but are not limited to field trips, community service events, physical training, transportation in district/government vehicles, participation on Drill Teams, Color Guard, Rifle Teams, and various competitions, fund raising projects, and other activities supervised by MCJROTC instructors. In consideration of permission extended by the government of the United States, through its agents, and or officers to take such passage, I do hereby and forever discharge the Government of the United States, all its officers, agents and employees, acting officially or otherwise, from any and all claims, demands, actions, or cause during any said passage thereto and any accidents occurring as a result of participation in the MCJROTC program. TRAINING REQUIREMENTS -  I understand that my child will be required to train and participate in all aspects of the MCJROTC curriculum which includes wearing of the MCJROTC uniform, classroom instruction/testing, marching and inspections, marksmanship training and physical fitness conditioning/testing. UNIFORM REQUIREMENT - I understand that each cadet is required to wear the Marine Corps uniform at least once per week on a day designated by the instructors.  The wearing of the uniform constitutes a significant portion of the cadet's grade.  I further understand that my child will be required to meet the basic Marine Corps grooming standards for MCJROTC cadets.  I acknowledge that failure to wear the uniform or abide by the MCJROTC grooming standards will most likely result in my cadet failing MCJROTC and possible dismissal from the program. I understand that my son/daughter will be issued uniforms, books, and other equipment whose value may be in excess of $300. I further understand these items are issued free of charge, but remain the property of the government and must be returned to the proper authorities when he/she leaves the program. I voluntarily agree to reimburse the government for the value of any such items issued to him/her which are lost, destroyed, or damaged through carelessness or misuse. Parent must provide initials. *

MEDICAL INSURANCE - I understand that the United States Marine Corps recommends that each cadet participating in MCJROTC be covered by either school medical insurance or private insurance at the parent’s expense.  This medical insurance should cover any and all injuries that may occur either on or off the school campus while participating in MCJROTC sponsored activities.  Please check one of the following: *
