Please fill out this form each day before your student(s) entering the facility for classes. The door monitor will use this form to check in all students. If the form has not been filled out for a student, they will remain with the door monitor, and the parent will be called and asked to complete the form before the student will be admitted to their classroom.

If your students will be absent from class, please also text their instructors.
