Starting April 7, 2023, Ohioans who are 17 years and older can receive a state ID card at no cost from the BMV. In light of this policy change, this form is to request birth certificate vouchers only.
It is Westminster's policy to assist with Birth Certificates once in a lifetime, no more than two per family. A program/charity may request assistance for a maximum of 5 clients per week.
This form is for caseworkers representing in-patient clients only. If your client is able to come to our program in-person on Wednesdays 10:00 a.m. - 12:30 p.m. they will receive their voucher the same day.
Caseworker voucher requests received by Tuesday 1:00 p.m. will be processed no by Friday of that week. Requests made after Tuesday 1:00 p.m. will be processed by Friday the following week.
