Place: Conyers, GA 30012 (Pickup address will be sent with confirmation once all documents are approved for pickup.

Pick up event will be outdoors, please be prepared and aware!! MASKS ARE REQUIRED!!

*** If you cannot pick up one of the above date/time/location combination... Do NOT submit this registration!!!
*** If you are not the parent or Legal Guardian...DO NOT submit this registration!! You will not be allowed to pick up!!
*** Your children MUST be present to receive toys!! If your child cannot or will not be present, DO NOT submit this registration!!!
(FYI...The toys might be visible to children & may NOT all be in a bag)
You must use a valid email address and be able to access it!! If you are unable to access the email address you use you will not be able to receive pick-up instructions or your request confirmation that you will need to have access to and This is the way you will be notified if you are approved to pick up toys for your child/children on Christmas Eve or newly added day: Christmas Day
If approved:
You will receive an approval to pickup email if your documents are received and we have toys available in the appropriate age group of your children.
You will then receive an email or text message that you MUST reply to confirm. You will then be sent a confirmation message that you must bring (electronic of paper) to pick up your children's assigned toys.
We apologize for the short turnaround, our organization was not scheduled to pick up toys until December 18th. The toys then had to be sorted according to age to ensure there's age appropriate toys available for your child/children and prepared to distribute.

Email the items below after you submit this request and must be received before the request is closed. If the request form is still open, even on Christmas Day we will still review your request for completeness and toy availability.
1. Birth Certificates for each child
2. Proof of Income (Paystub, Income Statement)
3. Recent TANF Letter (If you receive TANF)
4. Your Photo ID (Parent of Legal Guardian),
Bring on Pick Up Date:
1. Child(ren), (bring on pick up date
2. Your confirmation email. (bring on pick up date)
Please follow ALL of these instructions so that we can proceed with your request. If information is left out we will NOT fulfill your request as time is of the essence.
Submit only 1 (one) toy registration per family household & include all of the children in your household of whom you are parent or legal guardian.
To update your response: If you need to make changes or corrections, return to the email confirmation you received to update your response.
