Parent's Day Out is an RSVP Only Event. Please fill out this form so we can adequately prepare to care for your child(ren)!

**Even if you have filled out the medical information on a previous childcare event form (like Tough as Nails), please complete again to be sure we have the most up to date and accurate information.**

During the event, we will have a short scheduled snack time. HTF will provide Pirate's Booty and/or Skinny Pop. Please select your choice below:

Due to food allergies, we will bring our own snack. (NOTE: please label both food and drinks with child's first and last name and bring in a lunchbox or bag).
