WCSSAA Outdoor Event Sign-in
Welcome to this WCSSAA sporting event.  We are asking all visitors to sign in to assist with contract tracing.  Information shared in this form will be used by the School Administration and Waterloo Region Public Health if needed.  

Before attending any WCSSAA, please complete the Ontario School Screener: https://covid-19.ontario.ca/school-screening/

While attending this event, please maintain physical distance (2m/6ft) when near people who do not reside in your residence and wear a mask properly at all times.
