St Mary's Church Youth Club Form (Oasis)
Dear Parents/Guardians/Carers,

This is the registration form for Oasis (St Mary's Church Youth Club). Sessions will be held every 2nd and 4th Sunday of the month from 12:30pm- 2pm in the crypt. If you would like your child to join please complete this form. More details can be found below.

Time: 12.30pm- 2pm
Location: St Mary's Church Crypt Rooms
Ages: 11-16 years old

Please note: You must complete a registration form before attending.
