Bollywood Dance Central 4-Class Package Enrollment Form
This form is for enrollment in a 4-Class Package with Bollywood Dance Central. Please fill this form out once per enrollment. For instance, if you have two children both of whom you would like to enroll in 4-Class Package fill this form out twice. Note that a 4-Class (4 hour) Package is $96 at all locations and will be automatically renewed as a subscription every four classes. If you wish to cancel your package, it must be done prior to your last class date.

Liability: I/we realize that participation in dance classes and activities could involve some possible personal injury. Despite precautions, accidents and injuries may occur. By signing this release form, I/we (the dancer and parent/guardian) assume all risks related to the use of any and all spaces used by Bollywood Dance Central. I/we agree to release and hold harmless Bollywood Dance Central including its teachers, dancers, staff members, and facilities used by both entities from any cause of action, claims, or demands now and in the future. I/we will not hold Bollywood Dance Central liable for any personal injury or any personal property damage, which may occur on the premises before, during or after classes. Furthermore, I/we agree to obey the class and facility rules and take full responsibility for my/our behavior in addition to any damage I/we may cause to the facilities utilized by Bollywood Dance Central. I understand that Bollywood Dance Central is a licensed and insured business. In the event that I/we should observe any unsafe conduct or conditions before, during or after my/our classes, I/we agree to report the unsafe conduct or conditions to the Owner, Director, Instructor or Staff member as soon as possible. *
I understand that a 4 class package covers 4 hours of class time. Each hour counts as an individual class. If I attend multiple hours in one day, it will count as multiple classes. I also understand that I am signing up for a subscription of 4 classes that will renew when I complete my 4 hours of class. I understand if I opt in for performances, extra hours of class time may be required and performance days will count as a class. *
