Cancellation/Hold Request Form 
CANCELLATION POLICY: 
All membership cancellations must be received by the 15th of the month prior in order to not be charged on the 1st of the month. You will receive an email confirmation of the cancellation.  Dues paid will not be credited or refunded. All cancellation requests MUST be submitted via this form.

HOLD REQUEST POLICY:
All monthly memberships may be placed on hold for up to 4 months per calendar year for a $10 hold fee.  A confirmation email will be sent to you once the hold is activated.  Hold requests must be submitted at least 24  hours before your hold begins. When a hold is removed your account will resume auto billing. You MUST submit this form in order to request a hold on your account.

REMINDER: 
Please keep in mind that we have a no show/late notice cancellation policy. You may cancel your enrollment for free up to one hour before the start time of your chosen class. After that you will be charged a $10 late cancellation fee. If you're unable to attend please unenroll as soon as possible so that our instructors can plan accordingly.
