If you will be/are attending Hawkeye Community College or Allen College but living in the residence halls at the University of Northern Iowa (UNI), please complete this form to submit an Accessibility Accommodation Request for housing, dining, or parking accommodations and begin the process of getting connected with Student Accessibility Services (SAS).

The purpose of this form is to assist SAS in providing reasonable accommodations. The information in this request will be treated in accordance with the Family Educational Rights and Privacy Act (FERPA).

Documentation assists SAS in understanding how the disability impacts the student in an postsecondary education setting and the current impact of the disability as it relates to the accommodations requested. SAS will review and consider all pieces of information submitted. SAS takes into consideration the student’s personal narrative of their experience of barriers, past accommodation history, and supporting documentation related to their condition when making accommodation determinations. Reviewing documentation is a collaborative process and is handled on a case-by-case basis.
