1. PROPER AUTHORITY. I am the parent or legal guardian of the participant(s) named below, or I am an authorized adult that has obtained permission from the parent/legal guardian of the participant(s) named below to execute this Agreement on their behalf. I further represent that the participant(s) is/are healthy and physically able to participate in any and all activities.

2. ASSUMPTION OF RISK.  I understand that I/the participant(s) am participating in martial arts activities that may include classes, training, instruction, open mat, seminars, camps, events, programs and competitions in the martial arts school and other facilities being utilized for the martial arts activities. I understand that these activities may expose me/the participant to, without limitation, direct contact with and/or close proximity to others; direct contact with objects such as, without limitation, equipment, fixtures, furniture, glass, floors, mats, gear, bathroom facilities, water fountains, machines, clothing, and merchandise after entering any facility where these activities are being conducted. As such, I/the participant(s) understand that participating in these activities exposes me/the participant(s) to the risk of serious bodily injury and/or death as a result of exposure to influenza, viruses, bacteria, and other contagious illnesses and diseases.

         On behalf of myself and the participant(s), I voluntarily, freely and expressly choose to assume and accept all risks associated with the use and activities covered by this Agreement, and I understand that those risks may include personal injury and/or death.

3. GENERAL RELEASE FROM LIABILITY.  On behalf of myself and the participant(s), I fully and forever release and discharge TKADojo, LLC, together with their present and former employees, officers, directors, principals, agents, heirs, executors, administrators, predecessors, successors, assigns, and representatives from any and all claims, demands, actions, causes of action, contracts, obligations, suits, debts, costs or liabilities, whether known or unknown, which I ever had, now have, or may hereafter claim to have against TKADojo, LLC.

         I also hereby state that the students named below are physically fit to take the prescribed course of instruction and do so of their own free will. Occasional photos or videos of class or special events may be taken. Such materials may be used in advertising / social media. The undersigned waives claims to all model rights whether active or passive.

4. COVENANTS NOT TO SUE. On behalf of myself and the participant(s), I agree that I shall not institute any suit, action, or proceeding at law, in equity, or otherwise against TKADojo, LLC which I or the participant(s) may have by reason of injury to person, property, or death that arises from the use and/or activities covered by this Agreement, whether caused by the negligence and/or other fault of TKADojo, LLC, its agents, apparent agents, servants or employees, or from any other person or cause. I further agree that my heirs, executors, administrators, personal representatives, and/or anyone else claiming on my behalf or the participant(s) behalf, shall not institute any suit, action, or proceeding at law, in equity or otherwise against TKADojo, LLC, that I or the participant(s) may have by reason of injury to person, property, or death that arises from the use and/or activities covered by this Agreement.
