Please submit a separate form for each individual day of rehearsal being missed. Remember, conflicts should be communicated AT LEAST two weeks in advance. We recommend submitting all known conflicts prior to the season starting. All performance conflicts must be submitted prior by May 13th of the season and approved by Mr.s Angoli (angolem@masd.k12.wi.us)
