Dear Parents,
Thank you for your interest in the Headfort Summer Academy. In order to secure a place for your child, please complete the form below and make payment via bank transfer. It is vitally important that the Ref HSA (child's name) is included with all transfers. Please make full payment (€150 per week) with 10 days of completing this form to secure your child's place. After 10 days the place will be reallocated.

Please complete 1 form per child attending.

All children must be have completed Junior Infants- 14 years old.
