After you fill out this nomination request we will submit it to our selection committee. Bouquets go out the second week of every month.  You will be notified if your nominee is selected. Unfortunately, due to the high number of requests received we are unable to let you know if your nominee was not selected.

Part of the nomination form is a short survey (6 questions) to help us learn what our clients need and look for in a florist so we can better serve them.  We will NOT sell your information.  Your answers will NOT affect our decision on choosing nominees.
