Onsite supervision will continue to be provided until the end of the current restrictions.

Please submit this form as soon as possible before your child attending. Onsite attendance will only be authorised for:

*Children where both parents (where applicable) are 'essential' workers. Please email your work permits to office@sjmalvern.catholic.edu.au

*Extremely vulnerable children in out of home care and children known to child protection and other agencies.

*Children with a diagnosed disability where a parent or carer indicates that a student with a disability cannot learn from home due to vulnerability or family stress.
