Please complete this form if you would like to participate in the Go Lloyd Sidewalk Chalk Art Festival. Registration is free. All artists will receive a collection of sidewalk chalk, which they can keep after the event. All artists will be eligible for prizes.

Festival will take place on Tuesday, July 23rd. Location: Hassalo on Eighth Plaza. Artist may check in and begin their work at 9 am and should aim to have their work complete by 4 pm. Prizes will be awarded at 5 pm, artists do not need to be present to win.

Teams are allowed, each team member should complete this form and indicate who their teammates are.
