Please read the following important information and sign this electronic form, stating that you have read and understand the important dates and requirements of your child's Year 2 sacramental year.

I have read and understand that my child must have the following items completed and met before becoming eligible to receive their First Reconciliation:
-baptismal certificate is on file with St. Ann's
-family is registered as parishioners with St. Ann's
-child has attended 9 Launch classes or has made up those lessons virtually to fully understand the sacrament they are about to receive
-child has attended the First Reconciliation workshop on 10/22
If you have fully read and agree that all above requirements will be met by the due date of 11/12/23, please type your full name and date on the line below, indicating your acknowledgment.

I have read and understand that my child must have the following items completed and met before becoming eligible to receive their First Communion:
-all items required for First Reconciliation were met
-child has attended 19 or more in-person Launch classes or 24 virtual Launch classes to fully understand the sacrament they are about to receive
-child has attended the First Communion workshop on 3/10
-child has completed 1 hour of community service and turned in their form
-family has registered for a First Communion Mass and has paid the sacrament fee of $35
-child has attended First Communion rehearsal
