Anonymous Stories/Experiences for Disabled Empowerment in Higher Ed Month:
The purpose of this form is to gather disabled stories in higher education. Stories can be positive or negative experiences that have occurred during your journey in academia. You are not limited to only one story, but please fill out the form again. These stories will be shared via tweet on the Twitter handle @DisInHigherEd

Story or experience topics may include (but are not limited to):
- Journey to higher education
- Accommodations
- Overcoming internalized ableism
- Self advocacy
- Experiences with lecturers/advisors/staff/colleagues/students
- Experiences of ableism
- Barriers in Higher Educations
- Reasons why you dropped out
- Success and celebration stories

The demographics question are optional and are simply so that Disabled in Higher Ed can ensure they are sharing stories and experiences that are representative of the Disabled in Higher Education community as a whole. Mandatory questions are marked with an asterix(*)

All stories will be read prior to sharing, to ensure there is no profanity or hate language, and to facilitate the Disabled in Higher Ed Team to add content warnings if needed.

Here at Disabled in Higher Ed, we take your privacy seriously, and we will share your story anonymously. Emails and other information are NOT recorded as part of this form.
