Please complete the following form to submit all required paperwork for your athlete to be eligible to participate in athletics for the 2023/2024 school year.  These forms include:  UIL Acknowledgement of Rules, Concussion Awareness, Sudden Cardiac Arrest, Steroid Agreement, and the SISD Athletic Handbook Acknowledgement of Rules. It is important that you click on each blue link corresponding with the documents listed above, read, and review before electronically signing.  If you have multiple student athletes, a separate form will need to be completed for each.
