Medical Liability & Release: In consideration of participating in the programs at Starstruck Studios I represent that I understand the nature of this activity and that I am (or student is) in good health, and in proper physical condition to participate in such activity(ies). I acknowledge that if I believe event conditions are unsafe, I will immediately discontinue participation in this activity. By enrolling the above registered student(s), I certify that he/she/they is/are in good physical condition and able to participate in the activities that have been or will be scheduled. I further understand that I have declared any physical problems or restrictions (including those of adults who are participating with or without a child) and have listed any mental or special custody situations that would be important for Starstruck Studios to be made aware.I fully understand that this activity involves the risk of serious bodily injury, including permanent disability, paralysis and death, which may be caused by my own actions, or inactions, those of others participating in the event, the conditions in which the event takes place, or the negligence of the releasees named in this Registration and Release Packet; and that there may be other risks either not known to me or not readily foreseeable at this time; and I fully accept and assume all such risks and all responsibility for losses, cost and damages I incur as a result of my participation in the activity. I hereby release, discharge, and covenant not to sue Starstruck Studios, its successors and/or assigns, its respective administrators, directors, agents, officers, volunteers, and/or employees, other participants, any sponsors, advertisers, and if applicable, owners and lessors of premises on which the activity takes place, from all liability, claims and demands, losses, or damages, on account caused or alleged to be caused in whole or in part of otherwise, including negligent rescue operations and future agree that if, despite this release, waiver of liability, and assumption of risk I, or anyone on my behalf, makes a claim against Starstruck Studios, its successor and/or assigns I will indemnify, save, and hold harmless Starstruck Studios, its successors and/or assigns from any loss, liability, damage, or cost which I may incur as the result of such claim. This includes my heirs who may act in my behalf. I understand and assume all risks involved in connection with instruction, rehearsal and training at Starstruck Studios including, but not limited to, risk of bodily injury occurring as a result of contact with other students, instructors, walls, equipment, floors, and other objects on the premises of Starstruck Studios. This includes any travel to or from any such functions for or on behalf of Starstruck Studios. I understand that any injuries which may occur are the sole responsibility of me, the parent/legal guardian of the above registered student(s). I further understand that I, as the responsible party, must carry my own insurance, and that all classes are taken at my own risk and I accept this responsibility. I, the minor’s parent and/or legal guardian, do consent and understand the nature of the above referenced activities and the Minor’s experience, capabilities and believe the minor to be qualified to participate in such activity. I hereby release, discharge, covenant not to sue and AGREE TO INDEMNIFY AND HOLD HARMLESS Starstruck Studios its successor and/or assigns from all liability, claims, demands, losses, or damages on minors account caused or alleged to have been caused in whole or in part by negligence or otherwise, including negligent rescue operations, and further agree that if, despite this release, I , the minor, or anyone the minors behalf makes a claim against Starstruck Studios, its successors and or assigns, I WILL INDEMNIFY AND HOLD HARMLESS from any litigation expenses, attorney fees, loss liability, damage, or cost Starstruck Studios may incur as the result of any such claim. I further understand that there will be no refunds given or make‐up lessons scheduled for any classes missed by student(s). I further understand and have received the Starstruck Studios Parent Handbook and agree to abide fully by its terms. I understand and do hereby grant permission to Starstruck Studios LLC to use the image(s) of my child(ren) listed above. Such use includes the display, distribution, publication, transmission, or otherwise use of photographs, images, and/or video taken of my child for use in materials that include, but may not be limited to, printed materials such as brochures and newsletters, videos, and digital images such as those on the Starstruck Studios website. I agree that these images may be used by Starstruck Studios for a variety of purposes and that these images may be used without further notifying me. I do understand that the child’s last name will not be used in conjunction with any video or digital images for marketing purposes. I acknowledge the contagious nature of COVID-19 and other contagious diseases and viruses and voluntarily assume the risk that I and/or my children may be exposed to or infected by COVID-19 by attending and participating and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 and other contagious diseases and viruses may result from the actions, omissions, or negligence of myself and others, including, but not limited to, employees, volunteers, and program participants and their families.
