The following information collected is for the library internal use only. We will process your application within 10 working days from the day after receiving the application. It is recommended to make an appointment before visiting. We can't provide library services to you if we are outside working.

當您填寫此表格後即可使用圖書館服務。請妥為保存借閱證及不可轉交他人使用，申請補發需繳付港幣$20。
You can now enjoy our service after completing this form. Please keep your library card properly and do not transfer it to anyone else for using. Application for reissue the borrowing card requires a fee of HK$20.
