By filling out this form, you are agreeing to the following terms and conditions relating to the use of school-owned technology:

1. Only the authorized student user will access the device for educational purposes as directed by Crestline EVSD instructional staff. The student will not install new software or programs will be added to the device. The student is not permitted to alter or delete any software and programs currently installed on the device.
2. The parent and student will be responsible for the care and protection of the device while in their possession. Any damage to the device will be repaired or replaced at the expense of the parent. (Board policy 6152.01 is not applicable)
3. The parent understands the following Crestline EVSD Board of Education Policies remain applicable when the student is in possession of, accessing and using the device: 7540.03; 7540.06; 5513;  6152
4. The Chromebook will be monitored and protected through district filtering and firewalls as outlined in the district technology plan.
5. The Chromebook will be returned on the first day school resumes after the pandemic closure or within seven (7) days of the last day of the school year, as  adopted by the Crestline Board of Education. (June 2, 2020)
