I understand that by typing my name on this form I am agreeing to the Athletic Waiver and Release of Liability for my child to participate in Basketball tryouts at Freedom Classical Academy.

I also understand that my child does not yet have a roster spot on any Freedom Classical team until he/she has been invited by the Athletic Director/Coach, and accepted an offer to play for Freedom.

To accept the offer to play for Freedom the parent must follow all the steps to register as outlined in the welcome email.
