READ BEFORE SIGNING

This is a binding legal agreement. As a participant in the activities of Play Sask, the participant acknowledges and agrees to the following terms.

Disclaimer
Play Sask is not responsible for any injury, damage or loss of any kind suffered by a participant during, or as a result of, any activity caused in any manner whatsoever including, but not limited to, the negligence of Play Sask.

Release of Liability
In consideration of Play Sask allowing me to participate, I agree:
a) To assume all risks arising out of, associated with, or related to my participation. The risk from participation may include:
- Injuries from vigorous physical exertion, rapid movements, and quick turns and stops.
- Bruises, sprains, cuts, scrapes, breaks, dislocations, and head injuries resulting from contact with other players, balls, or the ground.
- Additional risks associated with my travel to and from the event, and associated with non-competitive activities related to the event.
b) To be solely responsible for any injury, loss or damage that I might sustain while participating; and
c) To release Play Sask (and their officers, officials, agents, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable, owners and lessors of premises used to conduct the event) from liability for any and all claims, demands, actions and costs that might arise out of my participating, even though such risks, injuries, loss, damage, claims, demands, actions or costs may have been caused by the negligence of Play Sask.
d) To allow Play Sask to use any pictures/videos taken of me during the course of my participation in any Play Sask activities and use them in Play Sask's promotional materials including online content.

I acknowledge that I understand the terms of this waiver and that I am signing it voluntarily.
