This form is for office use and includes specific waivers, contact information to be used, guidelines on eligibility. The CHSM Family Registration is different from the player sign-up forms.
Athletes can only participate in CHSM athletics once they have completed the CHSM Family Registration Form and the Player Sign-Up form.
All payments are considered as an agreement to participate in CHSM athletics and considered final. Refunds are only offered when we are unable to make a team.
**VOLLEYBALL and BASKETBALL FAMILIES...
All CHSM Volleyball and Basketball athletes must meet NCHC eligibility requirements. Failure to meet the requirements, or have received Hardship approval from the NCHC will result in the players inability to participate in games until they are approved.
