Colorado State Law (Colorado Revised Statutes 25-4-902) requires that each student have a current immunization record in the school file.

Brighton Adventist Academy also requires a current annual physical and health record in the school file. Please obtain a current copy from your child's physician and include it with your registration papers.

If your child is not current on their immunizations or their physical exam, please visit your family physician prior to registration day.

Please include the following with your registration papers or bring to the school on registration day.

Copy of most recent physical exam

Copy of immunization record (if your child is enrolled in the Colorado Immunization Information System, a copy is not required).

If you choose not to vaccinate your child, you must have Colorado State Exemption Form signed by you and your child's physician.

New Student Birth Certificate Request

All students must have a copy of their birth certificate on file. Please copy and return with your registration forms if you are a new student to Brighton Adventist Academy.
