I understand that my dancer is a guest invited by a registered dancer at Heather's Dance Company. I understand that there is some risk associated with dance/tumbling/twirling and I release Heather's Dance Company, Heather Reed, her staff and assistants from liability in the event that my child is injured.

If my child has any physical, mental, or other limitations, allergies, or situations that the studio should be aware of, they are clearly listed below.
