FSCI: Instructor release and permissions form

FSCI aims to create a re-usable and re-purposable record of the Institute. Our intent is to collect as much material from courses and participants as is practicable and to make the materials publicly accessible. We intend to deposit course materials to an appropriate Open Access repository under the most recent Creative Commons Attribution (CC BY) license. Through this form we explicitly ask permission to do this, for any materials related to your course which are provided to the FSCI team.

For instructors; we hope you will be able to leave us with 1) a record of the materials used and 2) work that came out of the courses (when appropriate). You are not obliged to provide all the materials you use, particularly any that have third party copyright restrictions.  We ask you to warrant that the course materials you do provide are free of copyright encumbrances and to grant a non-exclusive, world-wide, perpetual, non-revocable license to FORCE11 to publish these materials under a CC BY license. If these conditions are not acceptable or practical for specific materials, please do not include them.

This form is meant to inform you of our intentions for the course materials, and help you address any potential rights issues in advance of FSCI. You will likely be asked to physically sign a paper form with the same terms when you arrive at FSCI, to be sure the agreement is legally binding.
