AOS98 Food Delivery
To All Parent/Guardians,

AOS98 will be delivering meals to our students as well as any other children in the household that are 18 and under under - regardless of whether or not the child attends school.

To receive food delivery for your student and household, please complete this form.

To receive breakfast and/or lunch delivered to your home, please fill out the form below. There is NO COST and you may change or cancel your food delivery at any time during the closure.  No proof of income or free/reduced lunch requirement.  ANY CHILD IN OUR AREA (18 or under) MAY PARTICIPATE.

We will deliver meals to your residence daily, Monday through Friday, between 11:00 AM and 12:30 PM using the bus runs.

Food is being prepared by our ServSafe food service staff in school kitchens.  Delivery may be made by a community volunteer.  We will limit contact as much as possible (ie leaving a bag on the porch).  Please note some items may be perishable so do not leave the items outside.

This form only needs to be completed ONCE per household for the duration.

If you fill out the form and receive food delivery and then change your mind, you may complete this same form and fill out the name, address, schools questions and then last question on this form to let us know that you want to end food delivery for the children in your household.

You may change your form after submitting it or,  you may contact Kyle Canada, Safety and Security Director, 207-380-7271, with your questions or changes.
