- This form should be filled up by the teen or young adult Abot Tala High or Launchpad member and not the parent.  

- Abot Tala follows a self-directed model of learning. The classes and different program features you'd like to be part of should be freely chosen by the AT High or Launchpad teen/young adult member.

- Since you have been given the power to choose your classes, we trust that you'll be responsible for honoring the choices you've made. Signing up for a class means you are making a commitment to your mentor and fellow teen members attending the class.

- Should you wish to switch classes mid-block, please make sure to get in touch with your assigned mentor.
