How did you hear about the Nielsen Fitness Studio?
I will not, under any circumstances, enter the studio if I am feeling unwell or exhibiting any of the following symptoms: I’m feeling unwell or exhibiting any of the following symptoms: fever, dry cough, tiredness, aches and pains, sore throat, diarrhoea, headache, loss of taste or smell, a rash on skin, or discolouration of fingers or toes, difficulty breathing or shortness of breath.
I will arrive no more than five minutes before my scheduled start time and depart at my designated end time.
I will clean, spray and wipe-down all equipment, surfaces and fixed objects I have touched with the supplied sanitizing cleaning supplies.
I understand that cancellations made within 24 hours will not be refunded and will do my best to change/cancel well in advance when necessary.
I understand that my participation in the studio community can be revoked at any time – for any of the reasons above or at the discretion of Nielsen Fitness.
I will not let anyone else enter the Nielsen Fitness office without the expressed permission granted by Nielsen Fitness *
