Please fill out this form for EACH animal you plan to bring to our grooming day.

Please drop off your pets in the morning on December 3. We begin accepting pets at 8AM. We will call when your pet is finished. Last pickup is at 5:30PM. Questions can be directed to brandon.callis@austinisd.org. Cash and Checks ONLY please, paid at drop off - Checks made out to Akins FFA. Thank you!

Please provide details on how you would like your pet groomed. All groomings automatically include a brush, anal gland expression, ear cleaning, nail trim, a bath with shampoo and conditioner, and a sanitary trim if necessary (removing hair on the belly and "privates"). *
Photos and Promotion - I allow photos, videos, etc. to be taken of my pet and used in promotion of the Akins Vet Center via print and electronic means. *

They have been adequately trained and are supervised, but they are still learning. They are semi-professional and will uphold high standards. You agree not to expect professional results and will release the students and Austin ISD from liability except in cases of gross negligence or mal intent. The students of Akins Vet Center eagerly await your pets and will provide the best care possible. *
