2022-2023 Religious Education Volunteer Interest Form 
This form is for St. Raymond Faith Formation in Mount Prospect, Illinois.

We are thankful that you are interested in the volunteering with Faith Formation Team. The 2022-2023 Religious Education classes for grades 1-8 will be on Wednesdays from 5:30 - 6:30 p.m. beginning in September. You do not need to have a child enrolled in the program to volunteer.

Please note: Adults 18+ will need to complete Safe Environment training through the Archdiocese of Chicago.
