2024-2025 REPLACEMENT PASS FORM
Replacement passes are $10.00 each cash or check. All replacement fees will need to be paid to the Transportation Department Directly. Once you have filled out the form a replacement pass will be printed and given to your student from the Bus Driver within 24 hours. You have 30 days from the time the replacement form is filled out to pay the $10.00 replacement fee at the Transportation Department Office. If payment is not paid your Students Transportation Service may be Suspended.

My digital signature authorizes SJUSD Transportation to transport my student to and from the assigned bus stop as printed on the bus pass. I also realize that my student must review and adhere to the rules and regulations for safe riding as per the Rider Rules and Transportation Safety Plan, which is available on our website and in the Transportation office. Please take a moment and review the available information with your students. With your help and the cooperation of all student passengers, we will be able to continue to provide safe transportation for all passengers. All students are required to display his/her school bus pass to the driver each and every day before boarding at bus stops and at the school loading zones. Students without a current bus pass shall be denied transportation services and will be referred for possible disciplinary action.
