OHMS COMPUTER USE POLICY SIGN-OUT AGREEMENT

I agree to the procedures and expectations in set forth in Responsibilities of Accepting a Laptop Computer, Laptop Use and Care Guide, MLTI Laptops: Usage and Repair Fees, as well as Board Policy IJNDB and regulation IJNDB-R1.

These can be found on the district web site in Schools > Oxford Hills Middle School

Once the computer leaves the school area, there is NO guarantee of a filtered network.  Computers are able to pick up UNSECURED AND UNFILTERED wireless networks in many communities at places such as libraries, town/city buildings, businesses, and private homes.

I acknowledge that during the time the computer is assigned to my student, I am responsible for all costs and fees incurred to repair the computer and/or charger that are not covered by warranty or the Protection Plan.
