Registration is complete upon Paypal payment received:
$175 for Classroom portion
$200 for Behind-the-Wheel lessons (can choose to wait to pay this later)

If your child uses a preferred name, we will use that name in class and for in-class materials.

Parent Approval for participation: I hereby give consent for my child to take Driver Education at SLP High School. I understand that my child will be taught by a driver education teacher certified by the MN Dept of Public Safety and Dept of Education. Cars will be furnished by Driver Ed Inc. for 6 hours of behind-the-wheel. There are no guarantees that students will pass permit or license test. If a refund is requested, there is a $30 processing fee the first year, $60 processing fee the second year. No refund after 2 years.       Note the following:     A. Students must attend all classes of the selected section.    B. The permit test requires identification in the form of a Birth Certificate, U.S. Passport or immigration papers.    C.No transportation is provided for the class. You must arrange pick up.    D. Students age 14 or older can take the class, but must be age 15 to take permit test.    E. There is a one-time gas surcharge of $20 for behind-the-wheel training. *
