Application form for International students applying for UPH International Nursing Program.

Read carefully the applicant qualifications below and make sure you understand them before checking the boxes *

Applicants must be unmarried, at least 17 years of age, and less than 30 years of age

Applicants may be of any religion but must respect the Christian mission of UPH and the Christian faith of our lecturers and other students. As part of admission process, applicants will be required to sign a statement

Applicants must be a high school graduates with above average scores. An official English translation of a high school transcript must be provided

Applicants must meet UPH's minimum standards for English proficiency. Applicants must provide official evidence of English proficiency with one of the following standards: 6.0 in IELTS / 70 in TOEFL iBT / 520 in TOEFL PBT / 110 in Duolingo (recommended due to its lower cost and availability)

Applicants must have a passport with minimum remaining validity of 3 years, but preferably 5 years. If the applicant does not currently have a passport, or has not yet applied for a passport, it is likely there is not enough time to complete the application process

Applicants must be able to complete the application process, including English test, passport, and high school transcript by June 16, 2024. If materials are not submitted by that time, the applicant cannot be considered
