The following documentation must be provided before enrollment is completed:
1.  Proof of Residency in Coppell school district - parent name must be on settlement statement/lease and utility bill
     One of the following may be used:
          a) signed settlement statement (you must have already closed on your home); or
          b) signed rental/lease agreement (by both parties - student's name must be listed in the occupant section)
2.  A current utility bill (containing the resident name and address) gas, water or electric
3.  Student's Certified Birth Certificate and passport (including page with parent names)
4.  Student's Social Security card (if applicable)
5.  Parent / Guardian Driver's License (Photo ID)
       a) Custodial parent will need to provide a copy of divorce decree (if necessary)
       b) legal guardian must provide court document or notarized paperwork
6.  Most recent Report Card
7.  Immunization records (Texas state law will not allow any student to start without this).  
8. Withdrawal papers from current school if registering for 2020-2021 school year.

****If you have other students to enroll in CISD, please go to the appropriate campus website(s) and complete a New Student Enrollment Request form for each student that will attend the other campus(es).
