You only need to complete this form if your child will be eating breakfast at school.
Please check the box of the days your child WILL be eating breakfast at school. If there is an emergency, and your child needs breakfast, please contact the child's teacher or the office.
Please fill out the form for the month and complete the form at least 24 hours before the first day of the month.
Breakfast cost per day: $1.40 (Cash/Check is accepted in the Primary Office OR Funds need to be added to your Skyward Family Access account.)
Lunch cost per day: $3.20 (Cash/Check is accepted in the Primary Office OR Funds need to be added to your Skyward Family Access account.)
