Conflicts of Interest
HoMEUW volunteers shall:
• Not knowingly take any action, or make any statement, intended to influence the conduct of HomEUW in such a way to confer any financial benefit on themselves, their immediate family members, or any organization in which they or their immediate family members have a significant interest as stakeholders, directors, or officers.
• Disclose all known conflicts or potential conflicts of interest.
• Announce if there is any potential for perception that the person’s judgment could be affected by that person’s unique interest in the decision on that issue. Examples of potential conflicts of interest include being employed by or serving on an agency’s board or advisory committee that is under question, or the potential for receiving privileged information that could be beneficial to them, family members, or any organization in which they or their family members have a significant interest.

If applicable, please identify any personal or professional relationships, outside interests, activities, inside information, or other items that could be perceived as a conflict of interest. List the name of each organization, followed by a brief description of your role or relationship (including, but not limited to, volunteer, staff member, or board member). *
