Background Check: As a condition of volunteering, I give permission to the La Vernia Grizzlies Youth Sports Association (LVGYSA) and/or Tackle Youth Football & Cheer Association (TYFA), to conduct a background check. I understand if appointed, my position may be conditional upon a satisfactory result of this check. I understand that no applicant will be denied volunteer status solely on the grounds of conviction of a crime with the exception of relating to child abuse or a crime of a sexual nature. All other offenses will be considered by the nature, the date, the circumstances, and the relevancy of the offense.
I further understand that all information gathered will be securely held until the end of my tenure as a volunteer at which point it will be destroyed. If I provide fraudulent information with the intent to deceive LVGYSA, and/or TYFA that will be grounds for immediate dismissal and/or disqualification from any volunteer position.
