By printing my full legal name below, I signify that I am the parent/guardian of the teen named in section 2 of this digital 2023 Nazboro Permission form, hereinafter, “my child,” and that my child has my permission to both travel with and participate in the activities of Nazboro Teen Ministries of Hillsboro Church of the Nazarene from December 31, 2022 through and including January 1, 2024.
I understand that some or all of the activities in which my child may participate may involve a certain element of risk to his/her personal safety, either from the activity itself and/or in the transportation to and from the activity. However, I wish for my child to participate in the activities despite the potential risks and wish to assume those risks myself, to the extent that his/her damages are not covered by the Church’s liability insurance. By signing this form, I, the undersigned, hereby waive any and all claims against Hillsboro Church of the Nazarene and any of its staff and all persons organizing and participating in the events, from all uninsured liability for any injury to our child or to his/her property arising out of my child’s participation in our organization’s activities. I release and promise to indemnify, defend, and hold harmless Hillsboro Church of the Nazarene and any of its staff and all persons organizing and participating in these events for any injury arising directly or indirectly out of the described activities or transportation to and from these activities, whether such injury arises out of the negligence of Hillsboro Church of the Nazarene, staff, event organizers, event participants, our child or otherwise.
In case of an emergency, I the undersigned do hereby authorize Ryan Donley or such substitute as he may designate as agent for the Undersigned, to hospitalize, secure proper treatment for, and order injection, anesthesia, x-rays, surgery, or dental diagnosis for my said minor child as deemed necessary during the period of time specified above. I hereby consent and grant Ryan Donley, full right and authority to act for me in any matter pertaining to the care or control of my child named above during the specified period of time. In addition, as parent/guardian, I shall be liable and agree to pay all costs and expenses incurred in connection with such medical and dental services rendered to the aforementioned child pursuant to this authorization.
I further understand that any weapons, knives, alcoholic beverages, drugs and any form of tobacco products are strictly forbidden and, if my child is found in possession of any of these, he/she will be immediately sent home at the parent/guardian’s expense. If my child is caught fighting, swearing, engaging in public displays of affection or being disrespectful to sponsored adults or participating students, they will be sent home at the parent/guardians expense.
