Registration and Agreement Forms

I understand that I must pay for all classes I register for at DMB Dance Productions. Tuition is due on the 1st of every month, cash or check only. If fees or tuition have not been paid by the 5th of the month you will be charged a $15.00 late fee. If balances have not been paid by the 10th a second $15.00 will be applied to your account. If at any time your account becomes 30 days past due you will be removed from all classes until your account is up to date. Late fees will also be applied to all costume fee balances that are 5 days past due. Unpaid fees may result in not being allowed to participate in class or performing in dance shows. I agree to pay DMB Dance Productions a onetime yearly non-refundable registration fee.
I understand that DMB Dance Productions does not issue refunds for any reason including missed classes, illness, vacations, religious observance or withdrawal. Any missed classes can be made up in another class during the season, subject to enrollment and availability. Refunds will not be given for costumes, gear, shoes or any other ordered item. I give DMB Dance Productions permission to use any photos, videos, and/or voice recording for promotional and publicity purposes.
