Parents/Guardians: The CLC Participation Waiver form will be required for your student to participate in extended programming at the Lincoln High School Community Learning Center (LHS CLC). BY COMPLETING THIS FORM YOU ARE MEETING THAT REQUIREMENT.
Make sure to fill out a form for every student that you are parent/guardian for that is participating in the LHS CLC programming. If you have more than 3 students please fill out another form.
