Hello, and thank you for commissioning me! After you've received your work from me and the commission process is complete, please use this form to give me an idea of how your experience was. I love hearing your thoughts! I do not keep a list of who is filling this out or which responses correspond to whom, so your feedback here is anonymous and won't affect how I interact with you.
