Please use this form to share unique concerns regarding your child’s academic performance or social/emotional health that could affect his/her class placement. We will try to honor considerations for a teacher that one of your previous children had. However, we could have changes in assigned grade levels for some of our teachers and placement may not be possible. If this happens, we may not be able to accommodate all requests.

I want to emphasize that beyond siblings, requests for particular teachers will not be honored. Thank you for taking time to help us place your child with a teacher that best fits his or her needs.
